Welcome to Rehab On The Road. Please take the time to read the below Terms & Conditions and the informed consent. If you accept and agree with the below then please date and sign at the bottom of this page. Your appointment may involve a physical examination as part of the assessment then subsequent treatment, therefore there may be some physical contact from the therapist. As part of the assessment and treatment you will be asked to; >Undress to your underwear or remove clothing to allow assessment of a specific body part to take place. >Perform certain exercises or test that may make your symptoms better or worse either in the clinic or when you leave. The aim of the treatment is to relieve symptoms but on occasions the treatment may seem to aggravate your condition. This is common and normal. Aggravation may last a couple of hours or a couple days. Some treatment methods may cause temporary localised bruising. Any patient under the age of 16 must be accompanied by a parent/guardian for the entirety of the appointment time. It is the responsibility of the patient to inform the therapist of all medication, medical history any implants. The therapist must be informed if there is a possibility you may be pregnant. During the process of your session you may be asked to perform exercises with equipment you haven’t experienced before. At any point you feel uncomfortable it’s important you tell the therapist immediately. You may ask questions at any time and also ask the therapist to stop the assessment, treatment or rehabilitation if you wish. By ticking the below box it will act as a virtual signature.
